Administrators who want guidance on creating fields on a form in Assurance.

Administration access is required.

You can create new fields for a Register Template in two ways. Either through the specific Register Template under the Fields tab or from the Registers and Forms Field Library.

1. Creating a Field in a Register Template

From the Registers and Forms Template:

1. Select on the Fields tab.
2. You will see all the active Fields already being used on the Template.
3. Select on the +New Field button.

1.1. Add Fields from Library

On the Add Fields from Library pop-up modal:

1. Existing Fields can be added from the Fields Library to the Register Template, under the Type to search for a field section. A list of available fields will appear when you select and type here. Select the field and select the blue Add button.
2. A New Field can be created for this Register Template and will be saved to the Field Library by selecting Create a New Field.

1.2. Creating a New Field

Within the New Field pop-up modal, the following details are needed:

• Name - this cannot be the same as any other previously created field.
• Instructions (Optional) - can be provided to explain the type of information the user is to enter.
• Type - the type of field needed to capture this information. This is a drop-down field, so you can select from the list of options.
• Other information and format fields may appear depending on the Type of field chosen.
• Format - depending on the Type of field chose, you may select the format of the field type.

Select the Create or Create and New button.

2. Create a field from the Registers and Forms Field Library

Open the Registers and Forms Field Library.

From the Registers and Forms Field Library, select on the +New Field button.

1. Create the new field, by following the instructions above in Creating a New Field.
2. The field will be added to the Registers and Forms Field Library, available to be added into any Register Template.
3. Add the new field into a Register Template, by following the instructions above in Add Fields from Library.
