Users who need to request updated public liability insurance credentials from a provider.
No elevated permissions are required.
This article explains how to request a renewed public liability insurance certificate from a provider.
1. Request renewed public liability insurance credentials
- Go to the form and open the Credentials tab.
- Ensure that a contact and provider have already been created. The questionnaire cannot be closed when requesting new credentials.
- Select the check box next to Public Liability Insurance.
- Select Request Credentials.
- In the pop‑up window, enter the complete by date and select Create.
- Copy the generated request URL and send it directly to the provider.
- You can review the message by pasting the URL into a new browser window.
- The provider must enter the new renewal date, attach the updated public liability insurance certificate, and select Submit.
- Submitting the form automatically renews the credentials.
