Users who need to request updated public liability insurance credentials from a provider.

No elevated permissions are required.

This article explains how to request a renewed public liability insurance certificate from a provider.

1. Request renewed public liability insurance credentials

1. Go to the form and open the Credentials tab.
   • Ensure that a contact and provider have already been created. The questionnaire cannot be closed when requesting new credentials.
2. Select the check box next to Public Liability Insurance.

3. Select Request Credentials.

4. In the pop‑up window, enter the complete by date and select Create.

5. Copy the generated request URL and send it directly to the provider.

• You can review the message by pasting the URL into a new browser window.
• The provider must enter the new renewal date, attach the updated public liability insurance certificate, and select Submit.
• Submitting the form automatically renews the credentials.
