Register or Form fields can be re-ordered from the Register/Form Template.

Steps
1) Access the template from the Registers/Forms section

2) Go to the Fields tab

3) Drag and drop the fields to the appropriate position, using the claw tool on the left of the field

Note: A field that is dependent on the answer of another field cannot be moved above the field it is dependent on. For example, if I try and move 'Student's Year level' above 'Who was injured?' which has the answer of 'Student' that 'Student's Year level' is dependent on, this will not work. The video above shows this.
