How to Add an Assurance Public Link Form onto Your iPhone or Android Device
A Public Link to an Assurance Form can be added onto your iPhone or Android device, so you can report incidents, feedback and hazards on the go.
1. Adding an Assurance Form onto your iPhone home screen

1. Navigate to the Public Link Form on your iPhone.
2. Select the share icon (square button with the arrow).
3. Select the option to Add to Home Screen.
4. The Public Link Form will now appear as an icon on your home screen.

1. Navigate to the Public Link Form on your Android device.
2. Select the menu icon (three vertical dots).
3. Select the option to Add to Home screen.
4. The Public Link Form will now appear as an icon on your home screen.
