To Set Up Checklist Questions in Assurance

1. Select Checklists in the top menu

2.  Select Checklists from the dropdown menu that appears

3.  Click the Key next to the Checklist you wish to add questions to

4.  Select the Questions tab

5. Click the +New Question button

6. You will then be given the option to either:

a) Select a question from the Question Library (this is a full listing of all of the questions you have used in the past across all of your checklists). To select this option:

1. Click the text box with the text "Type to search for a question". This will reveal a dropdown menu

2. Select the question/s you would like to include in this checklist

3. Click Add.

or

b) Create a new question. To select this option:

1. Click the Create a New Question button.

2. Enter the Question in the space provided beneath Name

3. (Optional) Include instructions if necessary in the space provided beneath Instructions

4. (Optional) Click the textbox beneath Category and select a Category from the dropdown menu that appears.

5. Click the textbox beneath Type. This will reveal a dropdown menu which will list the available question types you can choose from. Note: If you would like to create a multiple choice question, you should select the Single Select Option here.

6. Enable or Disable the Lookup functionality by checking or unchecking the checkbox beneath Lookup enabled.

7. Click the box beneath Display Columns and select the number of display columns from the dropdown menu that appears.

8. Click the Add Answers button

9. Type the answer in the window that appears

10. Click Add.

Note: Repeat steps b8-b10 if you would like to provide the submitter with multiple options. E.g. if this is a multiple choice question you can repeat step b8-b10 three times to provide the submitter with three answers to select from.

11. Click the Create button if this is the only question you would like to create for the time being or Click the Create and New button if you would like to continue creating additional questions
