How to Create a Body Map Field on a Form in Assurance
An adult or child body image can be added to a Registers and Forms Template using the 'Image Annotation' option, which is a drop-down answer on the Format field that becomes available once you select 'Short Text' as a field Type.
When adding a Short Text field as normal, you will see a 'Format' drop-down field appear.
How to enable the Body Map field
- Create a Short Text field, by selecting 'Short Text' under the 'Type' drop-down. See here for the steps to create fields.
2. Select, 'Image Annotation' from the 'Format' drop-down list.
3. From here you can either choose the adult image or child image, by selecting the left or right arrows.

How to use the Body Map field in practice
*Important note: It is important to note that if you want to report on 'Injury Body Part', this information will need to also be captured in a regular Single or Multi-Select field. It could also be captured in a 'Text' Short Text field, but this does not allow for trend reporting like Single or Multi-Select fields do.*
- When submitting a form via the public link, drag your mouse around the area where the injury occurred and write notes about the injury.
- Save your changes and submit the form.

Body Map - find the public link from the template and edit the form. This video shows editing an Injury Body Part Multi-Select field as well, which we recommend for reporting purposes.
The video below shows how testing the feature can be done using the 'Preview' button once you have added the field into the template.
To make changes or add new information to the body map field once the form is submitted:
1. Select the pencil edit icon on the "Details' tab. Note: You cannot edit the field without following this step.
2. Make the change. If editing the same area that was injured, click into the box identifying the injured area.
3. Make the change by selecting the down arrow on the far right and select 'Edit' from the list of options, or 'Delete' if you want to remove a comment.
You can also 'Add a reply' or 'Add tag' by clicking into the grey space in the pop up box and both functions add further comments to the selection. The 'Add a tag' currently has no unique feature and operates the same way as 'Add a reply'.

Select the rubbish bin icon to remove the whole selection.
